Loss Runs
To obtain loss runs, see the requirements below:

All requests must be faxed to our Shared Services Department
at (480) 281-0870 or mailed to our physical address above.

If the request is from the insured, it must be on their letterhead with their
signatures.

We can send loss runs to the General Agent. We can accept requests from a
Retail Agent if we have documentation from the insured that it can be sent to
the Retail Agent.

We can send loss runs to a cancelled General Agent as long as they own the
policy.

Please provide the policy number, if possible.
